Method The patient elevates the shoulders through abduction and then extends the elbows to the onset of pain and holds for several seconds. This is followed by the external rotation of the shoulders which is held for several seconds. The examiner supports the shoulders and forearm in this position while the patient flexes the elbows. Reproduction of symptoms is a positive finding and may suggest brachial plexus or cervical root involvement
